Pre History
He tortures damned souls causing them horrific suffering. He was created by Death to be his slave and companion.
Death gets bored without a little physical pain in his life - he's a masochist! and Pain gives him a constant supply of arthritis, rheumatism, and many other diseases.
Because Death is amused by Pain he has allowed him to join the tournament.
